Aesthetic Medicine Consultation

The initial aesthetic medicine consultation is a fundamental step in assessing each patient’s individual needs and designing the most appropriate treatment plan.

It is recommended for anyone who wishes to reduce or prevent the visible signs of ageing, address aesthetic concerns, or enhance their overall wellbeing through aesthetic medicine.

During the consultation, which lasts approximately 30 minutes, the doctor will discuss your reasons for seeking treatment, any physical or emotional concerns, and the results you hope to achieve.

A comprehensive medical history will then be taken to ensure that any proposed treatment is safe and appropriate for your health. It is important to inform the doctor about any medical conditions, current medications, allergies, or previous reactions to anaesthetics or other drugs.

Based on this assessment, the doctor will recommend a personalised treatment plan tailored to your individual goals and expectations.

Once the treatment plan has been agreed upon, your aesthetic physician will determine the recommended number of sessions and the appropriate interval between treatments to achieve the desired results safely and effectively.

botox

Botulinum toxin (botox) is a purified protein that has been used safely in medicine for decades. In aesthetic medicine, it is one of the most effective treatments for reducing facial wrinkles and restoring a smoother, more refreshed appearance.

It works by temporarily relaxing the targeted facial muscles responsible for repetitive expressions that, over time, lead to the formation of dynamic wrinkles.

Today, botulinum toxin is considered the gold standard for the treatment of dynamic facial wrinkles, including glabellar lines (frown lines between the eyebrows), crow’s feet around the eyes, and horizontal forehead lines.

It can also be used to create a subtle lifting effect on the outer eyebrows, which naturally tend to descend with age, as well as to improve the appearance of platysmal bands of the neck.

Dr Rosano also specialises in treating the lower third of the face, including downturned corners of the mouth and the fine vertical lines around the lips (commonly known as “smoker’s lines”). Using an advanced injection technique, he can achieve a subtle eversion of the upper lip without the need for dermal fillers, creating a naturally enhanced appearance.

Dermal Fillers

Hyaluronic acid fillers are minimally invasive aesthetic treatments that involve the injection of hyaluronic acid to restore volume, enhance facial contours, smooth wrinkles, and deeply hydrate the skin.

Hyaluronic acid is a naturally occurring substance found throughout the body, particularly in the skin, connective tissues, and synovial fluid within the joints. Thanks to its exceptional ability to retain water, it plays a key role in maintaining skin hydration, elasticity, and firmness.

Dermal fillers are available in a range of gel formulations with different densities and degrees of cross-linking, allowing each treatment to be tailored to the specific area and desired aesthetic outcome.

Common Uses of Dermal Fillers

1. Aesthetic Medicine

    1. Lip enhancement and volumisation

    2. Correction of facial wrinkles and folds, including glabellar lines, crow’s feet, nasolabial folds, and marionette lines

    3. Enhancement of the cheeks and jawline

    4. Deep skin hydration with skin boosters

    5. Non-surgical rhinoplasty (liquid rhinoplasty)

2. Dentistry and Oral Surgery

    1. Aesthetic support following dental implant treatment by restoring perioral volume

    2. Facial profile harmonisation in patients with malocclusion or bone resorption

    3. Improvement of post-surgical scars

In modern aesthetic medicine, only temporary fillers are routinely used. These products are gradually metabolised and naturally absorbed by the body over a period ranging from approximately 3 to 12 months.

The primary goal of dermal fillers is to soften the visible signs of ageing by reducing wrinkles, restoring lost volume, and correcting facial depressions and folds. They are also highly effective for enhancing the lips and cheekbones, improving skin quality, refining the contours of the nose and chin, and achieving a more balanced and harmonious facial appearance.

Whenever clinically appropriate, treatments are performed using a microcannula technique, which helps minimise tissue trauma, bruising, and the risk of complications while improving patient comfort.

Are the Results Permanent?

Hyaluronic acid fillers are not permanent. Once injected, the product is gradually broken down and naturally absorbed by the body. The duration of the results depends on the treatment area, the type of filler used, and individual patient characteristics.

The aesthetic effect is visible immediately after treatment and typically lasts between 3 and 12 months. Maintenance treatments can be performed periodically to preserve the desired results.

Skin Biorevitalization

Skin biorevitalization is a minimally invasive aesthetic treatment designed to regenerate and revitalise the skin, improving its elasticity, radiance, and overall texture. It is particularly effective in addressing the early signs of ageing, skin dehydration, and loss of firmness.

How Does Skin Biorevitalization Work?

Skin biorevitalization involves the injection of regenerative substances directly into the skin, including hyaluronic acid, nucleotides, vitamins, minerals, amino acids, and antioxidants. These active ingredients stimulate the natural production of collagen and elastin, helping to restore skin quality and vitality.

One of the key advantages of biorevitalization is that it not only improves the visible signs of skin ageing but also addresses the underlying biological processes responsible for them. Rather than temporarily correcting a single aesthetic concern, it provides multiple benefits by:

  • Deeply hydrating the skin by enhancing water retention within the tissues.

  • Providing antioxidant protection against the damaging effects of free radicals.

  • Stimulating skin regeneration by promoting cellular renewal and the production of collagen, elastin, and hyaluronic acid, improving skin firmness and elasticity.

The treatment consists of a series of superficial microinjections performed in the upper dermis using either the picotage or linear threading technique, depending on the patient’s skin type and clinical needs. The procedure is generally well tolerated and causes minimal discomfort.

Skin biorevitalization can be performed on the face, neck (including the horizontal “Venus rings”), and décolletage.

Who Is This Treatment Suitable For?

Skin biorevitalization is suitable for both younger and mature patients.

For younger skin, it helps maintain hydration, elasticity, and skin quality while slowing the natural ageing process through preventive treatment. For mature skin, it helps reduce visible signs of ageing, improve skin quality, and support normal cellular function within the skin’s physiological limits.

The treatment is also recommended for individuals who wish to improve skin elasticity without adding volume or altering their natural facial features. It is particularly beneficial before periods of increased sun exposure, following significant weight loss, or for smokers, whose skin may benefit from additional regenerative support.

Fractional Multi-Needle Radiofrequency with Vacuum

Fractional Multi-Needle Radiofrequency with Vacuum is an advanced, minimally invasive technology that combines radiofrequency diathermy, fractional microneedling, and a vacuum-assisted system to promote tissue regeneration and skin rejuvenation.

This next-generation treatment is used in aesthetic medicine to stimulate both the skin and the underlying tissues, improving skin quality, firmness, and elasticity.

To further enhance the effectiveness of the treatment, Dr Rosano combines microneedling with the topical application of exosomes – extracellular vesicles rich in growth factors, proteins, and messenger RNA (mRNA). These bioactive components support communication between skin cells and stimulate the skin’s natural repair and regenerative processes.

The heat generated by the multi-needle radiofrequency handpiece induces controlled dermal contraction and remodelling of the fibrous connective tissue. Adjustable needle penetration depth allows every treatment to be fully customised according to the patient’s skin condition and treatment objectives.

The addition of vacuum technology makes the procedure more precise and comfortable by gently lifting the tissue during treatment, improving energy delivery while minimising discomfort.

How Does the Treatment Work?

  • Radiofrequency Diathermy (RF)
A specialised form of radiofrequency that delivers controlled thermal energy into the deeper layers of the skin. The microneedles transmit RF energy directly into the tissue, stimulating the production of collagen and elastin while improving skin firmness, elasticity, and overall quality.
  • Fractional Multi-Needle Technology
Multiple fine microneedles create controlled microchannels within the skin, triggering the body’s natural wound-healing response and promoting collagen remodelling and tissue regeneration.
  • Vacuum-Assisted Technology
The integrated vacuum system gently lifts the skin during treatment, optimising radiofrequency penetration, enhancing tissue response, reducing discomfort, and promoting lymphatic drainage.

Indications and Benefits

  • Reduction of wrinkles and skin laxity

  • Treatment of acne scars and other superficial scars

  • Improvement of hyperpigmentation and uneven skin texture

  • Fractional skin resurfacing

  • Natural skin tightening through collagen stimulation

  • Faster recovery and reduced downtime compared with more invasive ablative procedures

  • Improved tissue oxygenation and nourishment through the combined action of radiofrequency diathermy and vacuum technology

HIFU (High-Intensity Focused Ultrasound)

HIFU (High-Intensity Focused Ultrasound) technology, or High-Intensity Focused Ultrasound, is a non-invasive treatment that uses focused ultrasound energy to stimulate tissue remodelling and regeneration.

Thanks to the patented Bubble Effect and maximum focalisation, the treatment is painless, highly effective, and faster than conventional techniques.

How does it work?

Focused ultrasound penetrates deep into the skin without damaging its surface.

The heat generated reaches the deeper layers of the skin, including the SMAS (Superficial Musculoaponeurotic System), the same anatomical layer targeted during surgical facelift procedures.

The temperature generated (between 60°C and 80°C) induces controlled thermal coagulation, stimulating collagen and elastin production at specific depths, improving skin texture, promoting tissue regeneration, and reducing skin laxity.

The SMAS (Superficial Musculoaponeurotic System), which contains the deep facial fat that gradually descends with age, contracts immediately without damaging the skin surface or the surrounding tissues, producing visible skin tightening from the very first treatment session.

Main Applications and Benefits

  • Non-surgical face and neck lifting.

  • Reduction of wrinkles and skin laxity.

  • Improved facial contours and jawline definition.

  • Brow lifting and restoration of cheek firmness.

  • Treatment of localised fat deposits (e.g. double chin, abdomen, thighs).

  • Non-invasive treatment with no downtime.

  • Natural stimulation of collagen production for a progressive lifting effect.

  • Visible results after 2–3 months, lasting up to 1–2 years.

  • Targeted treatment without damaging the surrounding tissues.

What is the Bubble Effect?

The Bubble Effect is a phenomenon that occurs during HIFU (High-Intensity Focused Ultrasound) treatment when high-intensity focused ultrasound is used to heat the tissues. This effect plays a crucial role in the way HIFU stimulates tissue regeneration.

How does the Bubble Effect work?

HIFU ultrasound generates pressure waves that pass through the tissues and concentrate energy at a precise focal point. When this energy reaches the target area, the temperature rises rapidly to 60–70°C.

The Bubble Effect occurs when these high-intensity waves cause the formation of microscopic gas bubbles within the treated tissues, particularly in the intracellular fluids. These microbubbles rapidly expand and then collapse.

The collapse of these microbubbles generates microscopic shock waves, producing controlled thermal coagulation in the deeper tissue layers (such as the SMAS, the muscular layer beneath the skin). This stimulates collagen production and promotes a lifting effect.

Why is the Bubble Effect important?

The Bubble Effect is one of the key mechanisms through which HIFU achieves its non-surgical lifting effect. The microscopic shock waves generated by the collapse of the microbubbles stimulate the production of collagen and elastin, helping to tone and firm the skin.

This non-invasive process stimulates the skin’s natural regenerative processes without damaging the surrounding tissues.

PLASMA – Dermal Sublimation Microradiosurgery Technology

Plasma technology in aesthetic medicine is a non-invasive treatment that uses the fourth state of matter – plasma – to improve the appearance of the skin without surgery. This technology is particularly effective for skin rejuvenation, non-surgical blepharoplasty, and the removal of various skin imperfections.

How does plasma technology work?

  • The device generates a plasma arc by ionising the gases present in the air between the tip of the instrument and the skin.
  • This plasma arc creates a sublimation effect, transforming the superficial tissues directly from a solid to a gaseous state without passing through the liquid phase.
  • The plasma energy stimulates fibroblasts, promoting the production of collagen and elastin while encouraging cellular renewal.

Main Applications

Non-surgical Blepharoplasty

Corrects drooping eyelids and reduces periorbital wrinkles without the need for surgery.

Non-surgical Lifting

Treatment of wrinkles affecting the face, neck, and perioral area.

Provides a tightening effect to firm the skin and improve its overall quality.

Removal of Skin Imperfections

Removes sunspots, keratoses, warts, skin tags, acne scars, and stretch marks.

Scar Treatment

Improves the appearance of hypertrophic, atrophic, and post-acne scars.

Benefits of Plasma Technology

  • Non-invasive: no scalpels, sutures, or general anaesthesia required.
  • Minimal recovery time: significantly less downtime than traditional surgery.
  • Natural stimulation: promotes collagen production and skin renewal.
  • Long-lasting results: the effects can last for up to two years.

What to Expect After Treatment

  • Small crusts will form in the treated areas and will fall off naturally within 5–7 days.
  • Swelling and redness (particularly around the eyelids) usually subside within a few days.
  • Strict sun protection is essential to help prevent post-inflammatory hyperpigmentation.

Chemical Peel

Chemical peeling is an exfoliating treatment that removes the outermost layer of dead skin cells, revealing a brighter complexion and skin that appears smoother, healthier, and more youthful.

Chemical peeling works through several mechanisms:

  • It stimulates cellular regeneration (cell turnover), accelerating the natural renewal process, which slows down as the skin ages.

  • It indirectly stimulates dermal fibroblasts to produce collagen and elastin.

  • It provides a direct antibacterial effect.

  • It helps regulate sebum production.

This chemical treatment can also be used to reduce acne scars, improve skin discolouration, and regulate sebum production in cases of excessive oiliness. Naturally, only the expertise of a qualified aesthetic physician can help determine the most suitable peel for your skin type.

In recent years, advances in pharmacological research have led to the development of a wide range of peeling formulations with varying levels of intensity, designed to address different skin types and concerns. The most appropriate treatment should always be selected in consultation with an experienced Aesthetic Medicine physician following a thorough skin assessment.

Visible results can often be achieved after the first treatment. However, depending on your skin type and the desired outcome (such as reducing acne scars, eliminating skin pigmentation, or softening wrinkles), a course of 4 to 6 sessions may be recommended to achieve optimal results.

How is the treatment performed?

The procedure is straightforward. The physician applies the peeling solution to cleansed skin using a brush, accompanied by a deep massage. Once the required application time has elapsed, the product is neutralised and removed. Within a few hours, the skin will begin to exfoliate naturally, as small flakes of dead skin gradually shed. Following a deep peel, the skin becomes particularly sensitive. Depending on the type of peel performed, your physician may apply a soothing moisturising cream or recommend a specific at-home skincare routine to support the healing process.

Thread Lift

Thread lift treatments are aesthetic devices used in Aesthetic Medicine to achieve a non-surgical lifting effect and improve skin firmness without the need for invasive procedures. These threads are designed to stimulate collagen production, providing a tightening and firming effect.

How do thread lifts work?

  • Thread lift treatments use fine threads made from biocompatible materials (such as polydioxanone (PDO), poly-L-lactic acid (PLLA), or polylactic acid (PLA)), which are inserted beneath the skin through tiny incisions or with the aid of fine needles.

  • Once positioned beneath the skin, the threads are gently tightened and anchored to lift the tissues and improve their firmness.

  • During the treatment, the threads exert tension on the tissues, stimulating the production of collagen and elastin. This natural regenerative process gradually improves the skin’s elasticity and firmness over time.
